The following supporting document booklets may also be useful as you prepare for your meeting:

All completed forms and a copy of your MA Annual Report, should be returned to the Diocesan Centre by 31st July 2025.

The Charity Commission Trustee Eligibility Declaration form(s) needs to be completed for new trustees and should be handed to your MA Administrator. These will be required by the Charities Commission.

Individual Church Office Holders in 2025:

No hard copy church officer forms will be distributed to churches in 2025, to ease the amount of paperwork passed around the Diocese.

We have instead set up an online Church Officer notification form which is available here.

Churches can use this new facility to notify the Diocesan Centre of their Church Office Holders directly throughout the year. This will ensure that churches can be kept up to date with Diocesan communications, including general notices or role specific statutory updates.

The online form can be used by newly appointed church officers or long-term church officers who need to inform us of a change in contact details. All notifications will be verified by the appropriate cleric or MA Administrator.
